Face to face is always the most productive option, but yes, via Skype of FaceTime is another great option.

Standard Rate is $95.00 per each one-hour session, with discount packages and sliding scale rates based on income considerations, etc.

Check, cash, visa or master-card, due at the time of service (There is a $20 service fee for returned checks)

Do you accept health insurance?

Since there are so many different insurance companies with differing plans, Formation Life is not set up to file with insurance groups. However, receipts are provided for plans who accept and compensate for Life Coaching or Pastoral Counseling services.

Can I make a tax-deductible donation to FLM?

Yes, we are set up as a standard non-profit ministry. Provide your mailing address and you will receive a statement at the end of the calendar year.
